Welcome to our practitioner communication tool designed to facilitate open communication between the practitioners at Port Moody Integrated Health and the healthcare providers who refer patients to our clinic. This innovative tool lets you communicate any relevant patient information directly to a practitioner at Port Moody Integrated Health, as well as develop a follow up plan for after the patient has been seen.

An integrated approach to healthcare means that all practitioners involved with the care of a patient need to stay connected and informed during the course of treatment. Once a patient you have referred has been seen, you will receive an email from the treating practitioner that outlines their findings and treatment and management suggestions. Arrangements for further follow up can then be made to ensure you are kept informed about how the patient is progressing.
